Subcontractors' Prequalification Form
All subcontractors are required to complete this questionnaire. The contents of this questionnaire will be considered confidential and used solely to determine your company's qualifications.
Please complete all sections of the form. If you find that a certain question is not applicable, please indicate this by typing NA.
This form has been designed using Adobe Forms. You will need freely available Adobe Acrobat Reader Software (version 7.0 or above) to complete it. Should you need to install it on your computer you can find it on the Adobe website here.
You can download the form by right clicking on the link below and then selecting 'Save target as' to save it on your computer. Once downloaded you can complete the form at your convenience (it can be saved intermittently).
You can attach any supporting documents such as your safety policy within the form itself by clicking on the 'paper clip' icon on the top menu bar and then clicking on the relevant section of the form. This should bring up a dialogue box from which you can select the file from your computer.
Once you have completed the form, please click on the email submit button at the end of the form. This will bring up your default email programme and attach the form to the email. Simply click on the send button to send the form back to us. Should this not work for some reason, you can manually open your email programme, attach the form and email it to prequal@pochins.plc.uk
